Many Form I-765 Applications For Employment Authorization Must Now Be Filed With USCIS Lockboxes

USCIS announced that beginning February 24, 2010, applicants submitting paper Form I-765 for many types of employment authorization must mail their application to the appropriate USCIS Dallas or Phoenix Lockbox facility, based on where the applicant resides. The lockbox facility will then forward the application to the correct Service Center for adjudication.

Note: Form I-765 may also be electronically filed (e-filed) with USCIS when submitted under certain categories. Consult the USCIS Web site for information on who is eligible to e-file Form I-765.


Prior to this change, paper Forms I-765 were filed directly with the USCIS Service Center that would also adjudicate the application. Until March 26, 2010, Service Centers will forward mail incorrectly filed at the Service Centers to the appropriate USCIS Lockbox facility. After March 26, 2010, applications will be returned to the applicant, with a note to send the application to the correct location.

Applicants filing a Form I-765 at a USCIS Lockbox facility may also elect to receive an e-mail and/or text message notifying them that their application has been accepted, by completing an E-Notification of Application/Petition Acceptance (Form G-1145), and attaching it to the first page of their I-756 application.

USCIS Phoenix and Dallas Lockbox mailing addresses for the following I-765 application types commonly used by NAFSA members:
  • (c)(3)(A), Pre-Completion OPT
  • (c)(3)(B), Post-Completion OPT
  • (c)(3)(C), 17 Month STEM OPT Extension
  • (c)(3)(ii), F-1 Internship with an International Organization
  • (c)(3)(iii), F-1 Seeking Off-Campus Employment Due to Severe Economic Hardship
  • (c)(5), J-2 dependent of an exchange visitor seeking employment authorization
  • (c)(6,) M-1 Practical Training (according to a 03/15/10 NAFSA liaison meeting with USCIS, an M-1 practical training I-765 should only be filed with a lockbox under the limited circumstances where it is filed without Form I-539; if filed with I-539, then the entire practical training package should be sent to the address governing I-539 filings.)
  • (c)(9), Pending employment-based Form I-485 applicant (unless filing concurrently with Form I-485, in which case the package should be sent to the I-485 filing address)
